0

Update the Code of Conduct contact points.

The doc used to refer to dknox@, who is no longer involved in
the project, and a non-existant group for appeals.

This CL replaces dknox@ with dpranke@, and removes the sentence
about appeals, since we don't currently actually have a defined
process for appeals.
