
Migrate callers of BrowserAccessibilityState::AddAccessibilityModeFlags
to ScopedAccessibilityMode. No functional changes for the most part
except for:
- Accessibility is now turned off for a WebContents when ReadAnything is
finished with it, except for the ReadAnythingPermanentAccessibility
study, which is a holdback study that retains the previous behavior.
- AccessibilityUIMessageHandler now clears global and per-page flags set
by the user upon destruction.
- BrowserAccessibilityState::EnableAccessibility now modifies the flags
only once -- removal of non-additive modes is done along with the
addition of new modes.
- WebContentsImpl::RecordAccessibilityEvents now clears mode flags that
were added for the duration fo the recording session.
- Recording accessibility events for a WebContents now only turns on
accessibility for that one WebContents rather than all in the process.

For historical reasons, there are some small top level directories. Now the guidance is that new top level directories are for product (e.g. Chrome, Android WebView, Ash). Even if these products have multiple executables, the code should be in subdirectories of the product.
