We allowed to configure DM serve URL for test with crrev/c/4395471. But the configured value is not actually picked up. This CL does this. Also removed the reference to policy service from policy fetcher. And the service policy no longer has the fetcher as a member. Instead, a policy fetcher is created at the beginning of policy fetch, and owned by the callback until policy fetch done.
